My Medicines My Health green medicine bag campaign My Medicines My Health green medicine bag campaign

CCGs are working with other NHS organisations across the North East in promoting, investing and implementing the My Medicines, My Health campaign. As part of this campaign practices have received a resource pack containing leaflets, posters and Green Medicine Bag.

The Green Medicine Bag campaign aims to encourage patients, in particular those over 60 years of age with a long term condition, to store all their medicines together- in the Green Medicine Bag. This has a multiple benefits across the healthcare system and also for patients, these include:

  • Patients maintaining familiarity  and managing their own medicines
  • Reducing waste
  • Quicker discharge & improves patient understanding of medicine changes
  • Reduces errors in prescribing by supports medicine reconciliation on admission and discharge

When speaking to patients, encourage them to keep all their medicines in a bag or container and take their medicines with them when they go into hospitals.

We, as healthcare professionals, need to be reinforcers to achieve behaviour change in relation to medicine taking. Please thank your patients when they bring their medicines in with them, even if you don’t have time to examine them closely.

For more information please visit the campaign website www.mymedicinesmyhealth.org.uk and follow and retweet messages on Twitter @NHSmymedicines.
